If you or your family member is on Mississippi Medicaid and needs a ramp or other accessibility changes at home, the right program is not the one most people search for first. Mississippi's Division of Medicaid (DOM), the state agency that runs Medicaid, offers an Elderly and Disabled (E&D) waiver, but that waiver does not list home modifications as a covered service. The waiver that does is the Independent Living (IL) waiver, operated day to day by the Mississippi Department of Rehabilitation Services (MDRS).
What Mississippi offers#
The IL waiver is a statewide home and community-based services (HCBS) waiver under Section 1915(c) of the Social Security Act. It serves people age 16 and older, with no upper age limit, who have a severe orthopedic and/or neurological impairment. Examples include a stroke, multiple sclerosis, muscular dystrophy, spina bifida, or amputation. Applicants must also require nursing-facility level of care. A physician must certify that level of care, and it must be recertified at least every 12 months.
This matters for older adults specifically. A senior 65 or older can qualify for the IL waiver, since there is no upper age cap. But it only applies if the underlying impairment fits this narrower orthopedic or neurological category. If you searched expecting a standard aging waiver, the E&D waiver is the one built for older adults generally, but it does not fund modifications. Its service list includes "environmental safety services," which is a different benefit and not an accessibility adaptation. For home modifications specifically, the IL waiver is the route.
On the IL waiver, the covered service is called Environmental Accessibility Adaptations (Home Modifications): physical changes to the home required by your plan of services and supports, needed for health, welfare, safety, or greater independence. This can include ramps and door widening. One firm limit applies across the board: modifications cannot increase the total square footage of the home.
Amounts and eligibility#

Financial eligibility follows the special income rule used for institutional Medicaid: income up to 300% of the Supplemental Security Income (SSI) federal benefit rate, which is $2,982 a month for an individual in 2026. Mississippi has a distinctive option for people over that limit: a Medicaid income trust, where you pay the excess income to DOM each month to still qualify. Other full-benefit Medicaid categories, including SSI recipients and the working disabled, can also qualify for the IL waiver.
Only DOM and MDRS decide whether you qualify, based on a comprehensive assessment of your long-term services and supports (LTSS) needs. Nothing here is a guarantee of eligibility.
